The information collected on these registration forms is required in order to allow the Olympian Swim Club (OSC) to provide programming for your Member(s). The information will be made available to authorized employees of the club within the scope of their roles and responsibilities, and to individuals working with your Member(s) on a need to know basis.
I, the swimmer, or (if under 18), the parent/legal guardian of the Member(s) in this family agree to have information contained in these registration documents used for club purposes, and my Member(s) name and any pictures or likeness may be used on the club's website, social media, or print, etc.
