I, the swimmer, or (if under 18), the parent/legal guardian of the Member(s) in this family acknowledge and accept the risks and possible injuries associated with the sport of competitive swimming.
I, the swimmer, or (if under 18), the parent/legal guardian of the Member(s) in this family will immediately remove myself/my swimmer from participation, and notify the nearest official, if at any time I sense or observe any unusual hazard or unsafe condition including but not limited to my Member(s) or anyone in our family experiencing COVID-19 symptoms.
